Just a couple of more tasks for you to verify your eligibility to claim your prize. You have until June 26 to submit your claim for your prize.

Terms:

US Citizens

who win more than $50 in a calendar year should self-report winnings to the IRS on Form 1040 Line 21.
who win more than $500 in a calendar year must submit to Replication Markets IRS Form W-9 so we can file a 1099-MISC if prizes exceed $600. Submit completed IRS form to AccountsPayable@jacobs.com with subject line “PayPal-SCORE-<your name>”. Jacobs distributes prize money for the Replication Markets project.

Non-US Citizens

must submit IRS Form W-8BEN to Replication Markets so we can file IRS Form 1042. RM may have to withhold up to 30% of the prize money, depending on tax treaties. Submit completed IRS form to AccountsPayable@jacobs.com with subject line “PayPal-SCORE-“. Jacobs distributes prize money for the Replication Markets project.
